All children attending Ewa Beach Preschool are required to submit before starting date:
Completed Form 14
A. Physical examination must be completed within one year before first entrance into school
Proof of current TB test
B. Must be completed within one year before first entrance into school
C. Immunizations
- 4 DTaP/DTP/Td (diphtheria/tetanus/pertussis) shots
- 3 Polio shots
- 1 MMR (measles, mumps, rubella) shot
- 3 Hepatitis B shots
- 1 Hib (Haemophilus influenzae type B) shot
- 1 Varicella (chickenpox) shot
Your child MUST be in underwear before attending our preschool.Licensing
The Department of Human Services Childcare connection unit monitors our program with health and safety guidelines, as well as child teacher ratios.
Class Size and Staffing
The staff may be comprised of director/teacher, teacher(s), assistant teachers, and aides/cleaning person. The school will follow the State of Hawaii rules as a requirement but will strive to exceed these standards set by the Department of Human Services, who governs all child and welfare programs.
Gender Discriminating Policy
It is the policy of EPEP to require strict compliance with Title IX of the educational Amendments Act of 1972 (title IX) and to provide educational opportunities and programs free from gender discrimination. We do not practice or condone discrimination on the basis of sex as described in the act. This compliance assures each student the right to participate fully in classroom instruction and extracurricular activities without being limited or impaired because of gender. |
